A FARNHAM man received a letter from NHS Surrey informing him of a new system that would improve the present system of health records, despite having died at an NHS hospital two weeks prior to the letter being sent.

Ian Shearer, whose father Douglas died at Frimley Park Hospital on March 15, said the need to replace the present record system was highlighted by this error.
